What this policy covers.

This policy covers this website and the forms on it, run by Youtherapy Wellness and Physical Rehab, 318 New Haven Ave. Unit A, Milford, CT 06460.

It does not cover your clinical records. Those are handled separately, under the clinic's own HIPAA policies and the Notice of Privacy Practices you receive as a patient.

In short: this page is about the website. Your chart is a different system with different rules.

What the website collects.

We only collect what you type into a form, plus some basic technical information about your visit.

The appointment request and contact forms ask for your name, email address, phone number, and a short description of your problem. That includes the area of pain, roughly how long it has been going on, and how it is affecting you.

If you tick the text message consent box, we record that you agreed to receive texts from us.

The benefits check form asks for more, because insurance verification requires it. That form collects your first and last name, date of birth, ZIP code, phone number, email address, insurance provider, member ID, and group number.

The pain check tool records only the answers you select. It does not ask for your name.

Our servers and service providers also log ordinary technical details, such as your IP address, browser type, the pages you viewed, and which page a form was sent from.

Why we collect it.

To contact you back about the request you sent. That is the main reason.

To check what your insurance plan covers before your first visit, so the cost is clear up front.

To schedule your appointment and send appointment related messages, including texts if you agreed to them.

To keep the website working, keep spam out of our inbox, and understand which pages people find useful.

We do not sell your information.

We do not sell, rent, or trade your information to anyone.

We do not hand your details to advertisers or data brokers.

We share it only with the service providers listed below, who help us run the site and contact you, and only where the law requires it.

Who else handles it.

Running a website means using outside companies for parts of it. These are the ones involved:

• Our website host and form service, which stores this site and receives the form submissions you send.
• Our scheduling and patient contact system, which holds your request so our front desk can call or text you back.
• An embedded map from Google, which loads when you view our location.
• An embedded reviews widget from Elfsight, which displays public Google reviews.
• Google Fonts, which supplies the typefaces the site is set in.
• Our email provider, which carries messages between you and the clinic.

These companies process information on our behalf and under their own privacy policies. Embedded content such as the map and the reviews widget may see your IP address when it loads.

Cookies and analytics.

This site uses a small number of cookies and similar technologies. Some are needed for the site to function and to protect the forms from automated spam.

We may also use general website analytics to see how many people visit, which pages they read, and where they arrived from. That information is looked at in aggregate. We are not trying to identify you personally.

Embedded third party content, such as the map or the reviews widget, may set its own cookies. Most browsers let you block or delete cookies in their settings.

How long we keep it.

Form submissions are kept as long as we need them to answer you and to keep a record of the request.

If you become a patient, your clinical records are kept under the retention rules that apply to medical records in Connecticut, not under this policy.

Text messages.

If you consent to text messages, we use your number for appointment and follow up messages only.

You can stop them at any time by replying STOP, or by telling anyone at the front desk. Message and data rates from your carrier may apply.

Children.

This website is meant for adults. We do not knowingly collect information online from children under 13.

A parent or guardian who believes a child has submitted a form should contact us and we will delete it.

Asking a question, or asking us to delete your information.

You can ask us what we hold, ask us to correct it, or ask us to delete it. There is no form for this and no charge.

Please note that requests to delete website information do not delete a medical record. Medical record requests are handled separately, under the clinic's HIPAA policies.

HIPAA and your clinical records.

Once you are a patient, the notes, evaluations, and billing information created during your care are protected health information under HIPAA.

Those records are kept in the clinic's own systems, not on this website, and are governed by the clinic's HIPAA policies and its Notice of Privacy Practices.

Please do not send detailed medical history through a website form. A brief description of the problem is all we need to call you back.

Changes to this policy.

If we change how the website collects or uses information, we will update this page and change the date at the top.

This policy was written to describe, accurately and in plain language, how this website actually works. It is not legal advice.

Youtherapy should have its own attorney review this page before launch, and again if the forms or the services behind them change.
