Why It Happens
Your head is heavier than it feels, and your neck holds it up all day. When something below stops helping, the neck quietly covers the difference. It is usually a mix of four things. Neck joints that stiffened and stopped sharing the movement. A mid back and rib cage that went rigid, so the neck turns for both. Deep neck muscles that faded out while the big surface ones took over and stayed tight. And a nerve root getting crowded, which is what sends symptoms into the shoulder blade, arm, or hand.
Which one is driving yours changes the whole plan. A rigid mid back and an irritated nerve root need different work, and guessing wastes weeks. So we start with a cause-first evaluation, one on one with a licensed physical therapist. You are never doubled up with another patient. The whole hour goes to your neck.

How We Fix It

Weeks 1–2
Pain

We restore motion in the neck and the mid back together, so the neck stops doing two jobs at once.
Weeks 6–10
Perform
The deep neck and shoulder blade muscles get trained to hold you up, so the tight surface ones can finally stand down.

Prevent
Your desk, your pillow, your driving position, looked at and adjusted. Then a short routine you will actually keep doing.
This is the third PT that I have been to, and not to disparage the others, this is far superior to what I’ve experienced. You are treated as an individual. The therapist is not juggling three patients simultaneously. It’s an individual-based PT program.

No. Connecticut has Direct Access, which means you can see a physical therapist without a referral from a doctor. Call us and book the evaluation yourself. If your particular plan wants a referral on file, we will tell you that up front and help you sort it out. Getting started does not have to wait on anyone else.
Often, yes. The joints and nerves at the top of the neck can refer pain up into the head, which is why these headaches tend to sit at the base of the skull and climb from there. If moving your neck brings the headache on, that tells us a great deal. We test for it directly at your evaluation rather than assuming either way.
Yes, and it should not be rough. Nothing gets cracked or forced that you have not agreed to. Most hands-on work for a sore neck is gentle and slow, with you telling us what you feel as we go. If something is too much, you say so and we change it. That is far easier when your therapist is watching only you.
We will not put a number on it before we have seen your neck. What you get instead is a plan with an endpoint. After the evaluation you will know which of the four phases you are starting in, what has to change to reach the next one, and what is left after that. No open-ended string of appointments.
